Spouses, children, friends and extended family members need current and helpful information as loved ones move in and out of the health care and long-term care systems. The Foundation will work diligently to assist consumers as they navigate the continuum of long-term care and seek health care services.
Because 80 percent of all long-term care is provided by family caregivers, the Foundation will seek to offer information and guidance to them as they work selflessly to care for family members, neighbors and friends.
Millions of disabled and older Americans receive a variety of support services from their community. The Foundation will seek to support the efforts of service providers as they collaborate with health care and long-term care providers to offer disabled and older Americans the highest quality of life.
Skilled nursing facilities, assisted living providers, home health agencies, etc. provide sub-acute and rehabilitative care and services to millions of disabled and older Americans. The Foundation seeks to offer information to long-term care providers on a variety of issues to assist them in providing the highest quality of care to their residents and clients. In addition, the Foundation will share policy data with policy makers regarding health care reform and long-term care.
